The medicine
Roughly 15 minutes for most men. Dissolving under the tongue skips the digestive delay a swallowed tablet has, though a heavy meal or several drinks will still push it later.
The long-acting component can remain active for up to 36 hours. That is a window in which a response is easier, not a continuous erection.

One or two drinks are generally fine for most men. Heavier drinking both weakens the result and sharpens the side effects, because alcohol and these medicines both widen blood vessels.
Sildenafil and tadalafil are individually FDA-approved medicines. A compounded tablet combining them is prepared by a licensed pharmacy and is not itself an FDA-approved product. We keep those two statements separate on purpose.
Getting a prescription
No. You complete a written health form, a physician licensed in your state reviews it, and they issue a prescription where appropriate.
In most states no. The written form is the basis of the review, though a few states impose extra telemedicine requirements and you will be told if yours does.

Safety

Headache, facial flushing, a blocked nose, mild indigestion and light-headedness. They come from the same widening of blood vessels happening elsewhere in the body and usually settle after the first few doses.
Chest pain during sex, a sudden change in vision or hearing, or an erection lasting beyond four hours. All three need immediate medical attention rather than waiting to see.
